Is there a mechanism to add custom and/or cache-control headers to Umbracco's response, on a page or document-type level?

Nope, you're best off creating a custom HttpModule to ensure that it's done.
Umbraco does have support for macro caching, but that'll only be handy with macros, not whole pages.
