I have a few sites (v4.6 and v.4.7.2). After installation they are running great without typing .aspx in the urls. But sometimes that feature stops working.
I haven't used the umbracoUseDirectoryUrls in the config file or setup rules about it. It just worked from the beginning. But then once in a while it stops working and returns 404 on any node request that doesn't have .aspx in it. Normally it works again after restarting the IIS.
Not sure about your particular case---and not sure why you would have seen extensionless URLs without having specifically configured them---but can I ask what happens if you DO configure the umbracoUseDirectoryUrls (and UrlRewriteModule blank preCondition) ??
