In the production site we have found few issues with a member login, when investicated its found that there are two entries in the cmsMember table with same login name, it should not have happened, I wonder how Umbraco allowed this and there is no unique key set for the LoginName as the membership works based on login name.
Which version of Umbraco are you using? I just checked in 4.8.1, it seems to check for duplicate loginnames when adding a new member and prompts you to change it.
However, the check probably doesn't exist in the API, so if you created the members programatically it would probably bypass the check.
