We have an Umbraco 4.6 solution for one of our clients. They noticed a strange issue when publishing content. They have publishers in two different locations - Sweden and Brussles. If someone publishes content in Brussels, that page and content will not be visable for hours.
I upgraded the solution to 4.9. This sadly didn't resolve anything. I did, however, find a workaround to the problem. If the user in Sweden, after content is published in Brussles, publishes the page, as it is visible in the content manager, then it will be visable on the site.
From all my trouble shooting, I can only deduce that its a caching problem. Its very very strange that its geographical however.

Its very possible thats what the problem is. I really hope I can hear an answer from someone regarding this, its rather a major bug.
