My habit has always been to logout, clear browser cache, then log back in again if you don't see your updated styles in the tinyMCE editor right away. Not always necessary, and not sure under what circumstances this is needed, but whenever i think i'm going crazy these are the steps I take to fix it.

1. make sure you define the styles in a stylesheet
2. connect this stylesheet to the Rich text editor datatype
3. create the h1-h6 as subnodes to the css created in step 1

For me it works if only I call stylesheet file TinyMCE ( U.6.1.3 )
