We use KnockoutJS and Umbraco all time for insurance quote platforms.

All Input fields generated dynamicaly from Umbraco nodes on this page:

https://homeinsurance.telegraph.co.uk

If need any help just ask. KnockoutJS is very powerfull stuff.
