When you've made changes of the url-rules, you have to republish the nodes by right-clicking and choose "Publish" and check the box that says "Publish <nodename> and all its subpages".
As Patrik suggested, use the context menu Publish option, just make sure you are on your sites root node (not the Content node) as the Republish entire site option on the Content node just reloads the internal memory cache from the disk, rather than republishing everything. Other than that, if you have a lot of nodes, be patient =)

I have an existing site with a lot of pages. And I have just changed the urlReplacing in umbracoConfig.
Is it possible to republish the entire site to match the new rules in urlReplacing?
