Deploy Umbraco 5 to shared hosting environment

What do I have to do to setup my shared hosting environment to allow Umbraco 5 to run?

I tried copying across my local instance and setting write permissions to various folders (ie AppData, Bin etc) but it is still giving me a generic error (as below), yet when I turn custom errors Off it still shows the same error. Is there are a list somewhere on what I need to do for Umbraco 5 to run it on a shared host?

An application error occurred on the server. The current custom error settings for this application prevent the details of the application error from being viewed remotely (for security reasons). It could, however, be viewed by browsers running on the local server machine.

Yes, MVC3, ASP.NET 4.0 and IIS7. It is a medium trust environment. Do I need to set folder permissions on any specific folders (I've tried a few). Also running integrated security.
