We're working on Umbraco 4.0.4.1 and once in a while some images disappear on our server. As you can see in the screenshots underneath, the thumbnail still shows in the content section so the db links are still ok but the initial image is gone in the media folder.
We encounter this problem on upload fields and in the media section.
Is this a known issue in umbraco or are we doing something wrong?
