An odd one. A node was created (with sub nodes) and published, however, it wasn't appearing anywhere on the site. After looking at the node Properties tab I noticed the URL of the item was simply "#". Checking the logs, whenever the node was clicked on in the CMS there was an error entry saying "Couldn't find any page with the nodeId = X" but the next entry was "Open".
i.e. It displays fine in the CMS, all the fields and values are there etc and publishing it appears to work but no URL is assigned... If I "Copy" the node it appears to be fine.

Deleted the umbraco.config file (and let Umbraco re-create it) - seems to be working correctly after that.
