I've been usiong Umbraco for some time now and have multiple versions of it running and never had a problem, however since I've been running Umbraco 4.8.0 I've had a problem where saved items would revert back to their previous state and then after time update again; in some surcumstances I'd even have a page 404 where I've just created a new page, even unpublished items will sometimes behave as if they're still published as they previously were, I was wondering if anyone else has had this problem and if they could possible tell me how to fix it? This problem is effecting all websites, not just those running v4.8.0, in some cases even v4.5.x is effected but only since ive been using v4.8.0.

Ive managed to temporarily fix those problems by going to:
mysite/umbraco/dialogs/republish.aspx?xml=true

Obviously this is not a viable fix because it should not be doing it to begin with. Any help would be greatly appretiated!
