Our Team is been currently working for prestigious client. In our project, Umbraco workflow has been implemented with all the basic feature provided for writer, editor restricted access. Writer won't have publish rights and system will send notification to editor for node/page for publishing.
Looking at this workflow client was glad but image was changed by writer in media section and saved. this change directly reflect in UI, as they expected content workflow for media as well.
Media doesn't have a publish option. It can only save and will be directly visible in the website. I think it's best to make this part of the workflow so people know that media will be visible directly. If you move media to the bin it is also still available in the website until it's removed from the bin.
Regarding your tweet. There isn't really something you can implement. You just have to teach the writers that if they change an image it will be visible directly so they have the same rights on images as editors.
