    Have you considered using Umbraco Forms? (http://umbraco.com/forms). It's not free but it is what we have been using for this exact purpose for some time now for many clients and it works great or are you looking to write something yourself?

    I don't know of any existing packages although it may be worth a search on Our Umbraco for contact form packages. I can't seem to get at the package search on my mobile and I have no access to a computer today.

    What is your level of .net and Umbraco experience? You may have to look at creating a surface controller and custom form to achieve this if nothing already exists.
