I've also run into this. Are you using the image cropper? There is already an issue for this: http://issues.umbraco.org/issue/U4-4989

Sorry I thought I'd deleted this topic. I hadn't emptied the recycle bin! Having said that it would be a good idea for security to move deleted media items that are in the bin to an area where they are not accessible.
You could just hook into the media deleted event and add you own archival for items in the trash. :-)
