I'm hoping someone here knows what this is - I can sometimes replicate this behaviour by creating some content with this sort of structure
landing page -> child 1 -> child 2 -> child 3
If I rename the parent landing page then in the redirect table I'm occasionally seeing a trailing slash (row 3):
When this happens, the redirect just 404's, but if I manually remove the slash in the table it works as expected.
Has anyone else seen this before? There is some reference to it here

And I do have addTrailingSlash=true in the umbracosettings.config, but I don't see how that would explain the intermittent nature of the problem.
