We upgraded to Umbraco v6.1.6 before christmas and everything seems to have been ok. In the last few days we have noticed that items in Media section were appearing multiple times. All the properties and ID numbers are identical but we have 3-4 duplicates of the same items.
I hve attached a screenshot below to give you an idea. There are around 150 images and files and they are all doing the same thing.
The main content tree seems to be fine and this only affects Media. Appreciate the help and if you need anyfurther info please let me know.
Could you share som more information about what version you did upgrade from and did you follow the documentation on how to upgrade and make sure you read the upgrade details for specific versions? Which 3rd party packages are installed and what specifications does your server have?
Does this happen on new uploads or does it just randomly happen with media the has been uploaded a while ago?
Have you tried to see if there is any usefull information in the /app_data/logs file?
We upgraded from version 4 to 6.1.3 around October last year which was a big jump. We followed the documentation on upgrading and everything seemed to be ok. We upgraded from 6.1.3 to 6.1.6 just before Christmas but havent experienced any issues since then.
Strangely this has only started happening over the past few weeks.
The people using the site use Courier extensively and we have noticed this seems to package additional pages/items that are unrelated when publishing to the Live site - One of the pages seems to overwrite a completely seperate page that then has to be re-couriered to fix We also noticed that when images are resized they create a new image with the dimensions tagged on the end these are not transfered during the Courier process. Could Courier be the problem? Appreciate the help

We've had some bugs related to sort orders not being stored properly on older versions of Umbraco. To fix your current media tree (which, weirdly, leads to this issue), check out the last comment in this thread: http://our.umbraco.org/forum/getting-started/installing-umbraco/47047-Duplicate-media-nodes-when-updating-from-umbraco-3-to-umbraco-6
