Running U 7.4.3 and Courier 2.52.4
I have followed the instructions here:

This fixes the issue - however, every few days it comes back again and I need to run the SQL fix.
Is there an additional problem here?

If you still see the issue in Courier 2.52.4 then I would recommend you to create a issue on the tracker http://issues.umbraco.org/issues/COU and describe your issue, so it can be fixed once for all :-)
