Umbraco v7.5.4

Hi, I'm trying to create a form that has in it (including but not limited to)

• Town
• Post Code
• Number

And then an option to say "Click to add an additional location" This would then bring up the same options and the user would be able to add another location. I then need this to be unlimited, so they can keep adding locations. Or maybe have a limit on it, say five loactions.

What's the best way to go about doing this?
