When a user submits a form, all form fields are cleared out stating that they're mandatory... even when they're filled out. Filling out the form again, the form finally submits.

How is the form setup in Contour? No custom code etc.?

No none at all. Just the OOB fields, I just inserted form in a page view here: http://www.rockfordil.gov/forms/report-a-pothole.aspx

Yes, with preview and live forms. I solved this issue my removing a conflicting RadWindow Control???
