So I went and created a template from a form that is going to be used a lot - exported it and copied it to the templates directory, and I set the form GUID to avoid any issues with primary key violations, and created a new form based off it.
However, this new form only had the first of about 20 - the rest were simply not present at all.
The first couple of fields are custom field types - when I remove them, I do get the fields, until the end of the next custom field.
Does any of the wiser heads than mine out there have any suggestions as to what might be going on?

Well I found the answer to this...
In fact, it works fine. It just takes a hard page refresh (an iisreset, followed by normal refresh, didn't do it) for it to actually display properly.
So problem = obsolete.
