I've had a real problem trying to resolve this.
I am using Umbraco 4.11 and attempting to use Courier 3.0.14.
I have created a form and it did initially work fine and still does locally.
However, when I deploy the site files to a test server I get the error below when attempting to visit the page with the form on.
Error loading MacroEngine script (file: MvcRenderContourForm.cshtml)
Strangely, the form works on our other test server and on the live server.
To render the form I have included the macro in a template like so
I can't, for the life of me, work out what the problem. I can't debug on the test server and no errors are being logged in the umbracolog table.
I am I missing something or is there something else I can investigate to resolve the problem?
Any help would be great.

Could you try adding ?umbdebugshowtrace=1 to the page where your form is placed to see if that shows some usefull information about the error?
Are the local and test environments setup the same way? Are all files deployed correctly etc.?
