Have you had a look within umbracoSettings.config which is stored in the Config folder of the site. There is a section called <disallowedUploadFiles> make sure .csv isnt within there.
That's just an initial thought what might be causing the issue.
Then it mentions under the file upload restrictions:
If you need to update the allowed filetypes you can take a look at the configuration file
(App_Plugins/PerplexUmbracoForms/PerplexUmbracoForms.config).
So I would start there, and see if you can't just add your file type to a list of some sort..

thanks for the suggestion. I have had a look in the config folder it .csv was not listed.
