As far as I know, this is an umbraco feature that you cannot override.
The best you can do is make your own plugin that does it better. You could for example copy the javascript and html from the umbraco source and alter it so that it works the way you want it to.

This is really something that should be fixed in Umbraco. Title-attribute should never be auto populated since it will be presented by screen readers and may cause unexpected behavior.
Stop editor from auto populating title field for files
Hello,
When I insert a link to a file in the editor it auto populates the title field for the link with the name of the file.
Is there a way to stop Umbraco from doing this? I want the title field to be empty unless I enter something there manually.
