Are you getting any errors in the logging? Maybe something related to permissions that aren't setup correctly?

Hmm strange. Does it also give a 500 error when visiting the URL without the ?width parameter?

Do you have any incorrectly configured redirect rules or custom routes you may have registered to do with /media?

If this is only happening on non-local environments, please double check if your deployments are happening fully. Check your server(s) where your files are stored and ensure that these images are stored there. I have dealt with an issue where my deployments were not happening in entirety.
