Our programmer ran the rebuild index and optimize index for our production Umbraco database. The indexing completed.

It generated a number of folder icons which appear to phantom icons as shown below next to the real images in some of these folders. (Icons originally were folders, but we changed them to those shown below so as not to confuse users. The icons do no resolve as actual images when you expand the folder.

We then ran this utility on the Umbraco instance in our staging environment with the same aberrant result
