You're right. Currently you can only create an image using the seperate create link. I did this because in the previous UI it was hidden on a differnt tab in the pop-up. Some users didn't knew about the create option that's why I moved it to a more noticable place. If you don't like it you can download the source code at http://damp.codeplex.com/ and change it yourself :).
Create a new image?
In the old yet another media picker, I had the option to create a new image if the one I wanted wasn't there. I am not seeing that anymore?
Is the only options now to 1) look for your image, 2) if it is not there, close that window and open another one?
I like the old UI much better, much more intuitive - it took me a few minutes of messing around with to even notice the seperate create link.
