I attempted to add a form and it works well with all browsers except for IE. It turns out that utilizing the date field yields an error in the js:

That should have been fixed in Formulate 3.3.4: https://github.com/rhythmagency/formulate/releases/tag/v3.3.4
Please try upgrading.
Excellent! Thank you for that, I installed 3.5.1 and the form works in IE with the date field.
