I don't see why not. Formulate comes with the field types I'd expect to be used in a survey (radio button list, checkbox, text, text area, button, and so on).
Also, you can have Formulate store submissions to the database, then you can export those submissions later to CSV. You can then open that CSV in Excel and do things like create charts.

Hi, I would to install your package (instead of Umbraco Form) but I also need to create surveys and mange the results.
