There have been a couple of changes in Umbraco Forms 4.4 with regards to file storage used with the Upload Field. I assume you both are using this version of Umbraco Forms.
Our package was built using Umbraco Forms 4.3 and changes in Forms 4.4 unfortunately break compatibility of our package with Forms 4.4 with respect to the upload fields.
We are looking at the required changes which seem fairly minimal. We hope to release a new (beta) version this week which should fix the issues with Forms 4.4.
When replying I was under the impression this was posted as a bug report to our Forms package (https://our.umbraco.org/projects/backoffice-extensions/umbraco-forms-on-perplex-steroids/), which had some compatibility issues with Forms 4.4. I have posted a new version there (1.6) which should work properly again with Forms 4.4. However, this has no relation to the PerplexMail package, unless you also use the PerplexMail Workflow.
Either way, please install version 1.6 of our Forms package to see if everything works again. It was tested with a new installation of Umbraco 7.5.6 and Forms 4.4 and files were saved properly. Let me know if there are any problems with the PerplexMail Workflow if you use it.

We could not quickly test version 1.6 of our package with Azure, but I have changed our File Upload types to inherit from the default Umbraco File Upload types hopefully they "just work".

The relative virtual path 'forms/upload/ed59fee5-3dcc-4673-ac2b-49217242a618/projects.docx' is not allowed here.
Anyone had this before?

I'm experiencing it now with Umbraco Forms file upload on an Azure web app. No changes to the site for a few weeks now.
Are you running an Azure web app as well?
