I've almost got this working, but when I view in the browser, all I see is a blank page. If I view source, it all looks correct. Actually, it does this in IE8. Firefox says This XML file does not appear to have any style information associated with it. The document tree is shown below. and displays the xml.
Not sure exactly, but I know that I removed some white space from the top of the document. Otherwise, just keeping hitting the monitor.... just kidding.

I know that if there is a whitespace or an empty line in the beginning of the template, it will give the errors you experienced.
Will post a sample template for all to use.
