On our production site, we plan to remove or restrict access to the /umbraco folder. This causes ImageGen to fail as it lives in the /umbraco folder.
Simply place the imagegen.ashx file in another folder and call it from there. As long as the imagegen.dll is in the /bin folder it will work.

To avoid this issue in future, ImageGen 2.5 (now released) places the imagegen.ashx file in the root of the site rather than in the /umbraco folder.
