my client is experiencing a bug where on pages using image gen every 2nd load or so or refresh they encounter the white box with the cross through it and the image hasn't loaded. I can't replicate it here and was wondering if you thought it could be a proxy thing or some caching thing?
Also be sure you're running the latest version of ImageGen (2.2.x) by visiting the /umbraco/imagegen.ashx?version url for your site. If you've got 2.0.1 then the issue is probably related to the caching bug fixed by 2.2.x.

Check NTFS permissions. Also try to navigate the image url directly to see the error message.
yeah navigating to them directly works perfectly but adding image gen into the equation unfortunately it doesn't
but then direct linking via image gen works as well.. it's an odd one!
it seems to be when there are say 20-30 images all coming out of imagegen
