This happens just on the first refresh after saving the node, if I try another refresh it comes fine.
Any ideas?

I have been using it for a while without this issue. I have tried to add the XmlResolver = null on the created XmlDocument. Could you try the new assembly and report back http://dl.dropbox.com/u/538620/Skybrud.Umbraco.RelatedPagesByTags.dll

Thanks all for your help, I used another way that found on the forums and it's pure xslt, easier for me to understand and debug.
