Yes I know it's an old build, but the client isn't willing to pay for an upgrade.
Yesterday the server ran out of disk space, causing the umbraco admin area to stop displaying the items under the content/member/media trees etc.
On investigating I saw that the cache folders, e.g. /media/5651/cache, were absolutely mammoth for some images, over 70,000 files in one alone.
I'm going through and zapping all of the cache folders at the moment and the admin system is back up again, but has anyone else heard of Imagegen generating this much stuff? I've never seen it happen on any other Umbraco sites other than the 2 on the server I'm working on presently.
Very large media cache folders in 4.0.3

You must read this complete topic: http://our.umbraco.org/projects/website-utilities/imagegen/imagegen-bugs/2982-[UPDATE]-Fix-to-caching-bug-in-201
I had the same problem a couple of weeks ago. That topic solved it. I used this to remove all the cache folders: http://blackpoint.dk/umbraco-workbench/tips-and-tricks/removing-cached-media-folders.aspx?p=1
