Sure can. For the front-end or back-office? I released a package last year called uBackChat for the Umbraco back-office, lets you chat with other content-editors who are logged in.
As far as I'm aware there isn't a chat package for the front-end. A couple of people were looking to hack my code to do it, but not sure how far they got with it.

(I have no interested in developing it further myself.)
