I just make a search in the package repository, and found three different packages, for MailChimp.
I must mention, that i haven't worked with any of these packages my self.
But the package with the name Mailchimp4umbraco includes, two default signup forms, but maybe you will get some problems to get it work with 4.7.0 and 4.9.0 because it is developered for version 4.0.x.
As i mention, you could get some problems using the mailchimp4umbraco, because it was made for Umbraco version 4.0.x, exsample 4.0.3 it´s an old version of Umbraco.
And I don´t think the package is compatible with a newer version.
So I think you should try woodbasechimpmailer, and see if it has the features you were looking for. But I haven't tried the packages my self.
Another opportunity could possiblybe using the NewsletterStudio, as Rich mention in his post. I have never used the Newsletter Studio.
But if you will try the Newsletter Studio I think you should take contact to Markus Johansson, I´m sure that he will help you if you have any question about the Newsletter Studio package..
I think it´s really nice that you share, you found of the new packages for MailChimp of MVC for Umbraco. I think i can help lots for people who use the Mailchimp.

On my site I would like af newsletter formular, but I'm not sure which way to go.
I would prefer to use MailChimp so I hope some of you can help me further on that front :-)

Maybe this could be the solution http://our.umbraco.org/projects/backoffice-extensions/woodbasechimpmailer but I don´t know if it comes with a signup form. But you could try to ask in the Package discussions, on the page for the package.
The last package I found in my search was http://our.umbraco.org/projects/website-utilities/xslt-mailchimp
