Any action must be written in either the Surface controller or through Jquery.
And it is added to your template the same way you do in MVC in your view.
Buttons/Links or any action objects completely depend on the requirement.
You could create a macro that simply contains the button (hooked up to whatever controller action that's required)... and then allow that macro to be inserted into the RTE.

You can add a link not a button.
That is if the link is meant to redirect to a different URL on click.
