We got error message: 'PasteWordDialog' is undefined then paste words can't insert into editor.
We find the difference between new version and previous version in pasteword.htm
4.0.3 version: <form name="source" onsubmit="return PasteWordDialog.insert();" action="#">
previous version: <form name="source" onsubmit="saveContent();" action="#">

During the upgrade the tinymce3 \umbraco_client\tinymce3\plugins\paste folder didn't get overwritten with the 4.0.3 updated javascript files.
