On local we have no problems inserting templates into newsletters but on our staging (and live) environments we get a 500 error when choose the template and clicking insert.

Nothing is shown in the log files and the response is "An error occurred" so no clue there.
We have checked that all files are copied up and everything looks fine.
I can't reproduce this so I would need more information. If you look with 500-response the developer tools - is there any clues in the error message there?

When you say "inserting templates" what feature are you referring to?
It's strange that it works in dev but not in live. Where are you hosting? Do you see any errors in the logs och the trace-files?
Are you sure that the system have read/write permissions to the folders where the template-files are located?

It turns out that this works on local and live but not on our identical staging environment.
Bothare Windows Servers running IIS so nothing special.
I have attached a screen shot showing when the error occurs. It's when the plugin tries to insert the template into the RTE.
