I've installed the package on version 6.16 running on IIS6.
I’ve mapped the .pdf extension to C:\WINDOWS\Microsoft.NET\Framework64\v4.0.30319\aspnetisapi.dll as per the IIS6 instructions. This matches the other mappings for .net
(I tried mapping it to the v2 version of aspnetisapi.dll but it didn’t work at all)
The back office appears to work fine and allows me to set permissions. It also working fine for a test pdf file that I’ve protected. I can view it if logged in with correct permissions and get redirected if not.
However if I now try to view a non-protected media item pdf file I get a “page cannot be found” error.
An update for anyone who may come across this post. Just to confirm that Richard sorted this problem for me with a new release and I am now a very happy customer!

Are you sure you didn't try a wildcard mapping? I have seen issues on IIS6 with that in the past?
Have only mapped the .pdf extension as shown in the screenshots...
